  • Are at least 18 years or older
  • Are no receiving other benefits on their Social Security

  • Haven’t been denied their disability application in the last two months.

What Information Do I Need to Provide?

The information you need to provide to the SSA can be divided into three categories: personal information, medical information, and work history.

Personal Information

You must provide your full name, place, and date of birth along with your Social Security number. If you’re married, widowed, or divorced, you’ll also  have to provide your spouse’s name, SSN, date of birth, place of marriage, relevant dates of death or divorce (if applicable). Other personal information includes bank details and names of minor children.

Medical Information

You must provide the contact details of a person who knows your medical issues and can assist in your application. In addition to this, you must provide comprehensive information about your injuries, medical conditions, and illnesses including, treatments, prescription, medical history, etc.

Work History

You must also provide details of the current year and the last year’s earnings along with the employer’s name and address. A work history of about the last 15 years must also be provided along with any worker’s compensation you may have received.
